Purchase a Directory of AzPTA Members

The AzPTA offers three options for acquiring a directory of AzPTA members for your direct marketing needs:

Purchase Mailing Labels
The AzPTA will produce mailing labels containing the addresses of record for our 1400 members (PTs, PTAs and PT/PTA students). The fee for the complete lists is $200, or $100 for Maricopa County. Click here to complete and submit the mailing label request and payment form.

Purchase an Electronic File
The AzPTA will create an electronic file (.txt) containing the addresses of record for our 1400 members (PTs, PTAs and PT/PTA students). This file is sent only to an established mail processing business ("mail house") and not to the purchaser as it is intended for a single use. Purchase the 2009 Directory of Physical Therapy (hard copy)
The AzPTA periodically prints a hard copy booklet that contains the names and addresses of record for all Arizona licensed PTs and PTAs (AzPTA members and non-members). The fee is $300. Click here to complete and submit the 2009 Directory of Physical Therapy request and payment form.
