As aging populations and evolving reimbursement models revise physical therapy practice, the high incidence of falls presents challenges and opportunities across rehabilitation settings. This presentation compares two clinical strategies for fall risk management in cardiac and pulmonary rehabilitation settings. The first highlights the initiation of a fall prevention program in a conventional outpatient cardiac rehab model, examining strengths and practical limitations. The second showcases an innovative, technology-driven approach implemented by an early-adopter clinic. In this model, movement screening test data informs a customized assessment and intervention program, with exercise sessions remotely delivered and monitored through a mobile app. By contrasting these two frameworks, we aim to demonstrate how technology-enabled tools can expand clinicians’ capabilities in fall risk screening, assessment, and intervention.
