APTA AZ Award Guidelines and Nominating Forms

APTA Arizona recognizes individuals with various awards to acknowledge the efforts of clinicians, educators, and leaders who promote and advance the role of the physical therapy profession.

APTA AZ award nominations may be submitted anytime, but we encourage people to submit them by August 15 each year. Nominations submitted after August 15 will be considered for the following year.

PT of the Year

A physical therapist who, in the past year, has demonstrated significant contributions in the areas of education, legislation, membership, practice, and public relations.

PTA of the Year

A physical therapist assistant who, in the past year, has demonstrated significant contributions in the areas of education, legislation, membership, practice, and public relations.

Pauline Wampler Service Award

To recognize and honor an individual who has made a significant, ongoing, and distinguished contribution to the Arizona Physical Therapy Association (APTA AZ) through volunteerism.
